This is an excellent opportunity for a Nurse Practitioner (NP) or Physician Assistant (PA) who enjoys building long-term relationships with patients and providing high-quality, comprehensive primary care.About the OpportunityAs an APP at Carilion Family Medicine – Colonial Avenue, you will work collaboratively with physicians, APPs, nurses, and other members of the care team to provide patient-centered care across the lifespan. The role offers the opportunity to practice with a high degree of autonomy while benefiting from the resources and support of Carilion Clinic's integrated health system.Key ResponsibilitiesProvide comprehensive primary and preventive care to patients.Evaluate, diagnose, treat, and manage acute and chronic health conditions.Order and interpret appropriate diagnostic testing.Develop individualized treatment and follow-up plans.Educate patients and families about disease prevention, wellness, medications, and treatment plans.Collaborate closely with physicians, nursing staff, specialists, and other members of the interdisciplinary care team.Maintain timely, accurate clinical documentation and support quality and patient-safety initiatives.Help create an exceptional experience for patients and their families. QualificationsStrong clinical assessment, communication, and interpersonal skills.Commitment to collaborative, team-based, patient-centered care.Primary care or family medicine experience is preferred; qualified candidates with a strong interest in family medicine are encouraged to apply. Additional experience may be required based on specialty.Licensure, Certification, and/or Registration: Licensed or eligible for licensure as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) in the Commonwealth of Virginia.
