FTE: 0.3, Shift: Days, Schedule: DaysPosition SummaryThe Care Coordinator Associate is part of an interdisciplinary care team consisting of physicians and clinical professionals to assist in the cross continuum approach to hospital transitions. This position will be a key component to achieving collaborative practices that exemplify MultiCare Health System’s commitment to patient centered care, community engagement, and focus on transition of care for patients with physical and behavioral healthcare needs.The ideal candidate will have excellent customer service skills and a collaborative approach to ensure smooth throughput and care transitions for patients. The Care Coordinator Associate must be adept in technology to utilize multiple data platforms and troubleshoot as needed. In addition, this role requires organizational skills and familiarity with scheduling, insurance and authorizations.RequirementsPost-secondary coursework, preferably in a health related fieldBachelor’s degree highly desirableMinimum of 2 years experience in a health care setting, preferably as a Health Unit Coordinator (HUC) or in a clinical role such as medical assistant, certified nursing assistant or EMTExperience in utilization management, community resources, and/or financial and quality issues affecting patient care desirableAbout Good Samaritan Hospital MultiCare Good Samaritan Hospital is a 388-bed acute-care hub serving Puyallup and East Pierce County.
