Responsibilities include assessment, treatment planning, counseling, case management, crisis intervention, referral coordination, discharge planning, group facilitation, and other assigned clinical services.    This role collaborates with clients, families, referral sources, community partners, and interdisciplinary treatment team members to support engagement, continuity of care, and positive treatment outcomes.    Essential Duties and Responsibilities  Provide clinical services, including individual, family, and group sessions, and maintain communication with referral sources and outpatient providers as needed.  Conduct mobile behavioral health and substance use disorder assessments within assigned service areas.  Complete biopsychosocial assessments, clinical evaluations, intakes, and reassessments.  Assess clients’ behavioral, emotional, cognitive, social, environmental, and recovery support needs.  Perform ASAM assessments and determine appropriate levels of care in compliance with credentials, licensing standards, and regulations.  Develop admission, treatment, and clinical recommendations based on assessment findings.  Complete individualized treatment plans in collaboration with clients and the treatment team.  Collaborate with referral sources, hospitals, emergency departments, providers, families, and community partners regarding treatment and admission planning.  Partner with treatment teams to support service planning and clinical care needs.  Facilitate utilization review processes and maintain documentation to support authorizations and continued stay reviews.  Support aftercare planning, referrals, and continuity of care coordination.  Maintain timely, accurate, and compliant documentation in accordance with regulatory and organizational standards.  Participate in on-call rotations as assigned.  Maintain confidentiality and exercise appropriate prof...
