To learn more, visit www.habitathealth.com.Executive Director  Location: 702 North Azusa Avenue, West Covina, CA 91791 Role Scope:  Under the direction of Habitat Health leadership, the Executive Director is responsible for policy implementation, planning, development, administration, management, and program monitoring of the Habitat Health program.  Core Responsibilities & Expectations for the Role  Assumes overall accountability and responsibility for Habitat Health.  Supervises the Medical Director and other components of Habitat Health operations.  Ensures that the job responsibilities of all direct reports are defined and understood.  Responsible for development, implementation, and service delivery operations of the Habitat Health program.  Oversees budget development, monitoring, and compliance to ensure solvency and positive financial performance across all sectors of program operations.  Oversees changes in service delivery and staffing as required by development and growth of the program.  Oversees marketing, government, and public relations.  Oversees QAPI Program to ensure appropriate indicators are established and necessary outcomes are attained.  Coordinates creation of PACE provider application and coordinates completion of state readiness review and CMS on-site visits.  Assures the successful operational, financial, and clinical development of Habitat Health and the accrual of sufficient risk reserve funds.  Responsible for the development, implementation and operation of the Habitat Health program including establishment of PACE centers and all necessary participant services/programs, compliance with all applicable PACE, state and federal requirements and monitoring of all expenditures and expectations.  Responsible for fiscal management and operation of Habitat Health.  Disseminates information about the Habitat Health model; represent...
