Life Services Life Coach-Northern Region (Allen County or Lake County)

Job Summary:The Life Services Life Coach provides one-on-one coaching and development for participants in the Life Services program as defined by member needs both urgent, immediate, and long term to prepare members for employment or education programs and assist them with social determinants of health (SDOH) needs.Essential Functions:Complete required assessments by gathering information from the memberSupport a positive environment for the member by motivating, recognizing and engaging participants through proper communication and individualized practicesMaintain required documentation in case management system and follow all e-manual instructionsResearch developments in the fields of workforce and career development for those in and working to get out of povertyAct as a liaison to social service agencies (SDOH), government agencies, employers, employment and education programsAdvocate for Life Services participants to access resourcesCoordinate logistics for appointments as necessarySupport development of Care Plan and monitoring and reporting on participant statusArrange and coordinate services with community leaders; partners to maximize utilization of available resources and support groupsWork with CM to ensure Care Plan is carried out appropriatelyRegular travel to conduct member, provider and community based visits as needed to ensure effective administration of the programRequired to carry the minimum caseload set by supervisorPerform any other job duties as requestedEducation and Experience:Associates Degree in Social Work, Human Services, or Nonprofit Management Degree or equivalent years of relevant work experience is requiredBachelor’s Degree in Social Work, Human Services Administration, or non-profit Management is preferredA minimum of two (2) years of social service delivery experience is requiredExperience working with populations in poverty is preferredExperience with Motivational Interviewing is preferredCompetencies, Knowledge and Skills:Intermediate proficiency in Microsoft Office Suite to include Word, Excel and PowerPointDemonstrated coaching and counseling skillsAbility to influence and change individual behavior and assist individuals to achieve a high level of success and productivenessDemonstrated leadership skills to effectively develop, lead, inspire and motivate Life Services participants successfully navigate the “cliff effect”Ability to think analytically to identify qualitative or quantitative trends and take appropriate actionsDemonstrated change management aptitude and abilityExcellent written and verbal communication skillsAbility to work in a fast paced environmentAdaptable to a constantly changing environmentAbility to develop, prioritize and accomplish goalsAbility to work effectively independently and within a team environmentEffective problem solving skills with attention to detailSensitivity to and experience working with those in povertyAbility to separate personal from professional and to set boundar...
