Coordinate care between residents, providers, facility staff, and family members. Prepare residents for provider encounters and assist with visit workflows. Maintain accurate and timely documentation within the electronic health record. Support transitions of care, referrals, follow-up appointments, and other care coordination activities. Reinforce provider instructions and approved patient education materials. Support compliance, quality initiatives, and administrative functions that contribute to positive patient outcomes. Maintain reliable on-site attendance and professionalism while representing Theoria Medical within partner facilities. Your QualificationsMedical Assistants must have completed an accredited Medical Assistant program. Certified Medical Assistant (CMA) certification is preferred. Previous experience as a Medical Assistant in a clinical, post-acute care, care coordination, managed care, or geriatric healthcare setting is preferred.
