To learn more, visit www.habitathealth.com.Estimated Start Date:   ASAP Role Scope: We are seeking a Physical Therapist to join Habitat Health at our South LA PACE Center in a part-time capacity. The ideal candidate will work 16-24 hours per week. Specific schedule to be determined at offer. The Physical Therapist provides physical therapy evaluation and treatment across acute, restorative, and maintenance levels of care for PACE participants, developing individualized treatment plans, conducting home visits as needed, and educating participants and caregivers on exercise programs and DME use. As a core member of the Interdisciplinary Team (IDT), this role contributes clinical assessments and care coordination that inform whole-person care planning, while maintaining documentation that meets all regulatory requirements.
