You’ll be part of a multidisciplinary team providing hands-on, person-centered support across mental health, substance use, housing, and vocational needs—helping clients build stability and independence in real-world settings. Work directly in the community—no two days are the same Be part of a collaborative, mission-driven clinical team Help underserved populations achieve lasting recovery Engage in evidence-based, outcomes-focused care What You’ll Do Develop and manage person-centered treatment plans Coordinate care with a multidisciplinary team Conduct utilization reviews and ensure compliance with CARF, DHHS, and LME standards Lead and participate in treatment team meetings Support discharge planning and continuity of care Maintain strong communication with referral sources and stakeholders Complete timely, accurate clinical documentation Participate in CQI initiatives and peer reviews Provide community-based support, including transportation as needed Rotate in on-call/crisis response duties What We’re Looking For Bachelor’s degree in Social Work, Counseling, Psychology, or related field (required) Master’s degree in Human Services (preferred) Must meet QP status per 10A NCAC 27G.0104 1–3 years experience with severe and persistent mental illness (SPMI) preferred Experience in vocational or housing support is a plus Valid driver’s license Skills That Set You Apart Strong teamwork and communication skills Ability to build trust with diverse populations Solid clinical judgment and professional boundaries Organized, detail-oriented, and adaptable Training & Development We invest in your growth with ongoing training in: CPR & First Aid Ethics & Confidentiality Consumer Rights Case Management Continuous Quality Improvement (CQI) Why Pyramid Healthcare?
