Today, this includes going into individual homes as well as partnering with senior living communities to coordinate and manage clinical gaps for these seniors, their caregivers, and the care ecosystem. Savoy deploys a team of clinical experts both in-person and virtually paired with proprietary technology to drive clinical outcomes – without disrupting the current care.Work Schedule: Full-time, In-person in various senior settings with some opportunities to work from home part of the time. Position Overview: Savoy Life is seeking a dedicated Registered Nurse (RN) to provide hands-on clinical support for seniors in various setting including individual homes, assisted living, independent living, memory care and beyond to support direct patient care as well as extensive care coordination. The RN will work with residents, POAs, primary care providers, community groups, on-site senior living leadership to identify care gaps and implement chronic condition care plans to improve resident outcomes.
