Job DescriptionThis role is a fully remote position.  MUST BE LOCATED IN A STATE WE OFFER DEVOTED.A bit about this role:As a Transitions of Care (TOC ) Behavioral Health Social Worker, you will focus on Transitions of Care (TOC) support for Devoted members.  You will provide short term interventions to members in the process of being connected to community based behavioral health services, either because of an emerging need or following a hospitalization. The goal is to prevent avoidable emergency room/inpatient care and/or prevent readmission to the hospital.The TOC team is dedicated to providing culturally competent care with a focus on equity and quality.  We pay close attention to our team culture, treating each other like family so we have the requisite resiliency to care for our members like family.Required skills and experience: Master’s degree in Social Work with an active clinical license in any US state - LCSW, LMSW,  or comparable state clinical license.  Willingness to gain additional state licenses.Minimum of 5 years experience in the behavioral health or care management fieldsCompetent performing comprehensive psychosocial assessments over video and phoneAble to independently manage a fully remote work schedule, M-F, 8:30am-5:00pm PST or MSTDesired skills and experience: Bilingual in English and Spanish, Creole or Vietnamese preferred, but not required Experience in Geriatric BH or the health insurance industry (Medicare Advantage is a big plus)Your Responsibilities will include: Performing comprehensive psychosocial assessments via phone/video with members and caregiversConduct clinical social work interventions to maximize the effectiveness of the program including: brief therapeutic interventions, assistance in navigating family/support issues, assessing and intervening on social determinants of health, completing safety assessmentsInterdisciplinary collaboration with teammates (including clinicians, nurses, and care coordinators) to ensure cohesive, patient-centered care for high-needs patientsAbility to clinically identify and escalate complex members to the BH Medical Director for consultationActively participate in team huddles, panel reviews, interdisciplinary team rounds, and program/role growth and development Document all clinical interactions in the electronic health recordSalary Range:  $55,000 - $79,000 / yearThe pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting.
