At Cadre Hospice, we believe hospice care begins long before a patient enrolls. It starts with trusted relationships, meaningful conversations, and connecting patients and families with the care they need at the right time. We're looking for a passionate, relationship-driven Care Consultant to help expand access to exceptional hospice services throughout the community. You'll serve as the bridge between the community and our branch team by developing strong referral partnerships, educating healthcare professionals, and increasing awareness of the hospice benefit. Through authentic relationship-building and strategic outreach, you'll help ensure patients receive compassionate, high-quality end-of-life care when they need it most. If you're passionate about healthcare, thrive on building relationships, and enjoy making a meaningful impact in your community, we'd love to meet you. 🌟 What You'll Do 🤝 Build Meaningful Relationships Develop and strengthen relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, home health agencies, and other referral partners. Serve as a trusted resource by educating healthcare professionals on hospice eligibility, services, and the benefits of early hospice involvement. Build long-term partnerships founded on trust, collaboration, and exceptional service. 📈 Drive Community Growth Develop strategic territory plans to increase awareness of Cadre Hospice throughout the community. Generate qualified patient referrals through consultative sales and relationship-building techniques. Identify new opportunities to expand Cadre's presence and strengthen referral networks. Partner with branch leadership to support sustainable growth and increased patient access. 💙 Educate & Advocate Represent Cadre Hospice at community events, educational programs, and networking opportunities. Provide education to referral partners, patients, caregivers, and community organizations regarding hospice and end-of-life services.
