As an RN Case Manager, you'll serve as the primary clinical advocate for your patients, building meaningful relationships while coordinating personalized care that reflects each patient's goals, wishes, and unique needs. Working alongside physicians, social workers, chaplains, aides, volunteers, and other members of our care team, you'll help ensure every patient receives exceptional care with compassion, empathy, and respect. If you're passionate about delivering high-quality hospice care and want to work for an organization that values both clinical excellence and heartfelt service, we'd love to meet you. 🌟 What You'll Do 🩺 Deliver Compassionate Clinical Care Complete comprehensive nursing assessments to evaluate patients' physical, emotional, and psychosocial needs. Develop and manage individualized plans of care in collaboration with patients, families, physicians, and the interdisciplinary team. Monitor patient progress and adjust care plans as needs evolve to maximize comfort and quality of life. Serve as the primary clinical resource for patients and families throughout their hospice journey. ❤️ Manage Symptoms & Promote Comfort Assess and manage pain and other end-of-life symptoms using evidence-based clinical practices. Safely administer medications while monitoring effectiveness and potential side effects. Educate patients and caregivers on medications, symptom management, and disease progression. Proactively identify changes in condition and implement appropriate interventions. 🤝 Support Patients & Families Build trusting relationships through compassionate communication and active listening. Provide emotional support, education, and guidance during difficult conversations surrounding illness, grief, and end-of-life planning. Help patients and loved ones navigate goals of care while honoring individual values, beliefs, and preferences.
