Job DescriptionA bit more about this role: This position represents an amazing opportunity for an ambitious clinically focused physician ready to help build a virtual consult service with an interdisciplinary team. On a day-to-day basis, you will be responsible for clinical oversight to a patient panel across the country with an interdisciplinary team.Responsibilities will include:Deliver exceptional care to a cohort of complex patients across multiple geographies, leveraging video telehealth visits and a multidisciplinary team.Working closely with patients, caregivers, and community resources to empower patients on their journey to achieve health literacy, self-efficacy, and what matters most to them.Work with clinical leadership to develop and standardize team-based community practice, as well as leading your multidisciplinary team.Engage patients, patients’ caregivers/family, existing provider network & other Devoted programs to provide cohesive, comprehensive care experiences & optimal clinical outcomes. This will include working closely with patients’ non-Devoted PCPs & specialists, in addition to Devoted’s many specialized clinical & psychosocial teams, such as pharmacy, disease management & education, social work & palliative teams.Utilize our home grown electronic health information system (Orinoco) for visits while also providing feedback on how to improve the interface.Steward our patient centric and open clinical culture to our patients and colleagues.Attributes to success:You are a team leader, recognizing that everyone's success is your success.
