Depending on the focus of their team, Long Term Care Case Managers provide a variety comprehensive case management which involves providing individualized support and coordination of services for clients, often with complex needs. This includes conducting assessments to determine eligibility, creating tailored care plans, linking clients to appropriate resources, and maintaining ongoing communication to monitor progress and adjust plans as needed. It requires a thorough understanding of community resources, policies, and regulations, as well as the ability to advocate effectively for clients. Requires ability to meet individuals at their home and in the community to complete assessment and monitoring activities.   As a Children’s Case Manager, you’ll serve as a vital advocate and guide for families—helping them access services, understand their options, and create a coordinated care plan tailored to each child's unique needs. Experience in a high-volume customer service position demonstrates the ability to manage multiple tasks simultaneously while maintaining a professional and empathetic demeanor. This includes resolving client concerns promptly, addressing inquiries accurately, and handling challenging situations with diplomacy and patience.
