Our Assertive Community Treatment Team (ACTT) delivers intensive mental health treatment to adults living with severe mental illness—meeting clients where they are to reduce hospitalizations, improve stability, and support long-term recovery. Monitor medication compliance and treatment progress. Manage medication inventory, refills, and supplies. Maintain provider schedules and record client vitals. Provide medication education to clients and staff. Community-Based Client Care Travel throughout Madison County and surrounding communities to meet clients where they are. Transport clients to appointments and community resources. Help remove barriers to treatment and improve continuity of care. Crisis Response & On-Call Support Participate in rotating crisis phone coverage. Complete crisis planning and assessments. Assist with hospital diversion efforts. Support commitment procedures when clinically appropriate.
