Every Spring Health team member is expected to use AI tools thoughtfully, apply human judgment to AI outputs, and keep building AI fluency in ways that support their role and our mission.We're currently looking for an independently licensed mental health professional to join the CareNavigation team (LCSW, LMFT, LPC required) to support members seeking support withposttraumatic stress disorder (PTSD) and complex trauma. Reporting to the Lead Specialty CareNavigator of the Trauma team, the Trauma Clinical Care Navigator will provide initial assessment,support, referral, and ongoing care coordination for members within Spring Health’s innovativeTrauma program. This is a full time position that is fully remote. The shift for this position is Monday-Friday,9-5pm pst, mt or ct. What you’ll do:● Telephonic screening, brief intervention, and referral of members to a trauma specialtytreatment provider.● Apply active listening, psychoeducation, and motivational interviewing skills to understandeach member’s needs and provide support as they set goals.● Utilize Judith Herman’s phase based trauma treatment model to provide stabilizationsupport to members in their process of engaging in care.● Utilize motivational interviewing to assist members in engaging in care.● Identify an appropriate level of care, facilitate referral, and coordinate care with a SpringHealth provider.● Provide consultation regarding mental health concerns and family/relationship issues, asappropriate to member needs.● As necessary, provide in-the-moment support and safety planning for members withsafety, risks or crisis.● Utilize written communication skills to interact with members and providers through email.● Document information, maintain accurate records, perform follow-ups, and track progresswithin several systems, including an EHR within required timeframes.● Identify and problem-solve issues that serve as a barrier or disruption to care.● Interact with multi-disciplinary teams such as care support, provider operations, andbenefits/billing to ensure a smooth member experience.● Assisting other specialty care teams (SUD, Mood and Anxiety, and Eating Disorders), asneeded.● Participate in staff me...
